      Always login using your FormLink username and password from the login page at the following URL:

      https://palladium.formlinksystems.com

      1. If you have forgotten your FormLink password, you can select "Please click here if you have forgotten your password on the login screen".

      2. Verify your user name and select Submit to recover your password.Recover Password Submit
      3. Select Continue to exit the message window and return to the Login page.

       4. An email with a link for resetting your password is sent to your email account.

      5. Select the Reset my password link in the email.

      6. The Confirm Password Reset window is displayed. 

      7. Make note of the New Password and Select Continue.Confirm Password  Reset

       


      8. From the Login page, enter your Username new password and select Login.

      9. The Change Password window is displayed.


      10. Enter a New Password.

      11. Conform New Password.

      12. Select Change Password.

      13. Your password is now changed/recovered.

      FormLink User Tips

      • FormLink should be accessed using recent versions of modern mainstream browsers. This includes Mozilla FireFox, Google Chrome, Microsoft Edge (Chromium) and Safari on iOS.
      • Always access the FormLink login page using the URL that is available on FormLink's website, or a bookmark created from that URL. The bookmark may be needed to access FormLink in remote areas without internet access.
      • Always Disable Private or Incognito mode on your device.
      • Do not clear browser history or cache.
      • Devices should have automatic updates disabled. Updates can be applied after ensuring all locations and forms have been uploaded to the server.  In some cases, updates can clear the browser storage and cause a loss of data.
